AMOGAMMADEX
Generic Name: AMOGAMMADEX
ABSTRACT -
Amogammadex is a modified gamma-cyclodextrin type reversal agent being developed to reverse aminosteroid neuromuscular blockade, particularly from rocuronium and vecuronium. It acts by molecular encapsulation of the neuromuscular blocker, reducing free drug concentration at the neuromuscular junction. Because the molecule is a large substituted cyclodextrin, the representation above is a simplified cyclodextrin cavity schematic.
DOSAGE -
Dosage should be determined only by an anaesthesia professional.
Dosing depends on the depth of neuromuscular blockade, the neuromuscular blocking agent used, patient weight, and quantitative neuromuscular monitoring.
It is not a medicine for self-administration.
Patients require monitoring for recurrence of blockade, airway status, oxygenation, and hypersensitivity.
SIDE EFFECTS–
- Possible side effects include:
- Nausea
- Vomiting
- Cough
- Dysgeusia
- Bradycardia
- Hypotension or hypertension
- Residual or recurrent neuromuscular blockade
- Hypersensitivity or anaphylaxis
- Pain or discomfort related to intravenous administration
ALLRGIC REACTIONS–
- Hypersensitivity to amogammadex or formulation components may occur.
- Symptoms may include rash, flushing, urticaria, bronchospasm, hypotension, angioedema, or anaphylaxis.
- Important: Emergency treatment must be available during anaesthetic use.
DRUG INTERACTIONS–
- Other molecules capable of binding cyclodextrin cavities may theoretically affect reversal activity.
- Toremifene and fusidic-acid-type displacement concerns are considered for related cyclodextrin reversal agents.
- Hormonal contraceptive effectiveness may be reduced with related agents; follow local guidance.
- Anticoagulation status should be assessed when clinically relevant.
PREAUTIONS–
- Use only with quantitative neuromuscular monitoring where possible.
- Use caution in severe renal impairment because cyclodextrin complexes may rely on renal elimination.
- Ensure adequate ventilation until full recovery from neuromuscular block.
- Do not use to reverse non-aminosteroid neuromuscular blockers unless supported by evidence and protocol.
WARNINGS –
- Incomplete reversal can result in airway compromise.
- Severe bradycardia or anaphylaxis may occur and requires immediate treatment.
- Use only in anaesthesia or critical-care settings with resuscitation equipment.
- Keep out of reach of children.
