CADAZOLID
Generic Name: CADAZOLID
ABSTRACT -
Cadazolid is an investigational hybrid antibiotic combining oxazolidinone and fluoroquinolone pharmacophores, developed by Actelion for the treatment of Clostridioides difficile infection (CDI). Its dual mechanism — inhibiting bacterial protein synthesis (oxazolidinone) and DNA replication (quinolone) — targets C. difficile selectively with minimal systemic absorption, reducing gut microbiome disruption.
DOSAGE -
250 mg orally twice daily for 10 days for C. difficile infection in Phase III trials; taken with or without food.
SIDE EFFECTS -
- Nausea
- Headache
- Abdominal discomfort
- Flatulence
- Diarrhoea (paradoxically, as CDI treatment)
ALLERGIC REACTIONS -
These symptoms may indicate an allergic reaction and require immediate medical attention.

- Hypersensitivity
DRUG INTERACTIONS-
- Minimal systemic absorption limits major pharmacokinetic interactions
- Avoid concurrent QT-prolonging drugs (quinolone moiety warning)
- P-glycoprotein substrates: minimal local GI interaction possible
PREAUTONS–
- Not for systemic infections — designed for luminal CDI only
- Monitor for CDI recurrence (non-inferior to vancomycin in trials)
- Avoid prolonged exposure due to quinolone resistance potential
- Pregnancy safety not established
WARNINGS –
- Phase III results showed non-inferiority to vancomycin; development status varies
- Cross-resistance with fluoroquinolones possible in C. difficile strains
- Use of antibiotics associated with CDI recurrence risk in subsequent infections
