Generic Name: CAVOSONSTAT
ABSTRACT -
Cavosonstat (N91115) is an oral inhibitor of S-nitrosoglutathione reductase (GSNOR), an enzyme that depletes S-nitrosoglutathione (GSNO) — an endogenous bronchodilator and regulator of CFTR protein. By inhibiting GSNOR, cavosonstat increases GSNO levels, enhancing CFTR activity and airway smooth muscle relaxation in cystic fibrosis. Phase II trials evaluated it as an add-on to ivacaftor in CF patients with the G551D mutation.
DOSAGE -
Oral tablet; 100–400 mg twice daily studied in Phase II trials as add-on to ivacaftor.

- Nasopharyngitis
- Elevated liver enzymes
ALLERGIC REACTIONS -
These symptoms may indicate an allergic reaction and require immediate medical attention.

DRUG INTERACTIONS-
- Ivacaftor co-administration studied — monitor for additive LFT elevations
- CYP3A4 metabolised — interactions with strong inhibitors/inducers expected
- Avoid hepatotoxic agents concurrently
PREAUTONS–
- Liver function monitoring required — GSNOR inhibitors associated with LFT elevations
- Not a substitute for CFTR modulators in eligible patients
- Pregnancy safety not established
- Use in CF genotype confirmed amenable to GSNO-related CFTR enhancement
WARNINGS –
- Hepatotoxicity risk — LFT elevations can be clinically significant
- Phase II outcomes mixed — further efficacy data required
- Not approved — clinical trial use only
