CAVUTILIDE
Generic Name:CAVUTILIDE
ABSTRACT -
Cavutilide is an investigational antiarrhythmic agent being evaluated for conversion and prevention of atrial fibrillation (AF) and atrial flutter. It exerts dual ion channel activity, blocking multiple potassium currents (IKr, IKs) while activating the late inward sodium current, thereby prolonging atrial refractoriness. Phase II data suggested efficacy in acute pharmacological cardioversion of AF.
DOSAGE -
Intravenous infusion for acute cardioversion; dose and infusion duration determined in Phase II trials; optimal regimen under investigation in Phase III.
SIDE EFFECTS-
- QTc interval prolongation
- Ventricular ectopy
- Nausea
- Hypotension
- Bradycardia
ALLERGIC REACTIONS -
These symptoms may indicate an allergic reaction and require immediate medical attention.
- Infusion-related hypersensitivity
- Skin flushing
- Urticaria
- Anaphylaxis (rare)
DRUG INTERACTIONS-
- Avoid concurrent QT-prolonging drugs (antiarrhythmics, antibiotics, antipsychotics)
- Digoxin levels may be affected
- Additive hypotensive effect with antihypertensives
PREAUTONS–
- Continuous ECG monitoring required during and after infusion
- Correct electrolyte abnormalities (potassium, magnesium) before use
- Avoid in patients with pre-existing prolonged QTc
- Resuscitation equipment should be available
WARNINGS –
- Risk of proarrhythmia including torsades de pointes
- Use in monitored hospital setting only
- Contraindicated in cardiogenic shock or decompensated heart failure
