CAVROTOLIMOD
Generic Name: CAVROTOLIMOD
ABSTRACT -
Cavrotolimod (AST-008) is a spherical nucleic acid (SNA) TLR9 agonist formulated for intratumoral injection. TLR9 stimulation by unmethylated CpG oligonucleotides in plasmacytoid dendritic cells and B cells triggers innate immune activation, priming anti-tumour adaptive immune responses. When administered intratumorally in combination with checkpoint inhibitors, it may induce an abscopal effect. Phase I/II trials evaluated it in advanced solid tumours and cutaneous malignancies.
DOSAGE -
Intratumoral injection; dose in mg per injected lesion defined in Phase I protocol; administered in combination with pembrolizumab.
SIDE EEFECTS -
- Injection site reactions (erythema, swelling, pain)
- Fatigue
- Fever
- Chills
- Flu-like symptoms
ALLERGIC REACTIONS -
These symptoms may indicate an allergic reaction and require immediate medical attention.
- Local hypersensitivity at injection site
- Urticaria
- Systemic inflammatory response
- Anaphylaxis (rare)
DRUG INTERACTIONS-
- Additive immune activation with PD-1/PD-L1 checkpoint inhibitors
- Systemic immunosuppressants may blunt TLR9-mediated response
- Live vaccines: avoid during treatment course
PREAUTONS–
- Intratumoral injection requires image guidance for non-palpable lesions
- Autoimmune condition flares possible with TLR9 stimulation
- Effective contraception required
- Not for intravenous use
WARNINGS –
- Systemic cytokine activation may occur — monitor for fever and inflammatory responses
- Immune-mediated adverse events including checkpoint inhibitor irAEs may be amplified
- Not approved — use restricted to clinical trials
