CATUMAXOMAB
Generic Name: CATUMAXOMAB
ABSTRACT -
Catumaxomab (Removab) is a trifunctional bispecific antibody targeting EpCAM on tumour cells and CD3 on T cells, with an Fc region that additionally activates accessory immune cells (NK cells, macrophages). It received EMA approval for intraperitoneal treatment of malignant ascites in EpCAM-positive carcinomas. Market withdrawal followed commercial reasons, not safety concerns. It represents a pioneering trifunctional immunotherapy concept.
DOSAGE -
Four intraperitoneal infusions of 10 µg, 20 µg, 50 µg, and 150 µg on days 0, 3, 7, and 10 respectively, via an indwelling peritoneal catheter.
SIDE EEFECTS -
- Cytokine release syndrome (abdominal pain, fever, nausea)
- Vomiting
- Abdominal discomfort
- Fatigue
- Liver enzyme elevation
ALLERGIC REACTIONS -
These symptoms may indicate an allergic reaction and require immediate medical attention.
- Severe infusion hypersensitivity
- Anaphylaxis
- Urticaria
- Angioedema
DRUG INTERACTIONS-
- Systemic immunosuppressants may reduce T-cell engagement efficacy
- Concurrent intraperitoneal chemotherapy: interaction data lacking
- Live vaccines contraindicated during treatment
PREAUTONS–
- Administer in specialised oncology centre with ICU support available
- Premedication with analgesics and antipyretics recommended
- Monitor for cytokine release syndrome — particularly after 3rd and 4th doses
- Hepatic function monitoring mandatory
WARNINGS –
- CRS including severe abdominal pain and fever is expected and dose-limiting
- Liver enzyme elevations can be severe — monitor LFTs before each infusion
- Withdrawn from market — no longer commercially available; historical reference
