ASFOTASE ALFA
Generic Name: ASFOTASE ALFA
ABSTRACT -
Asfotase alfa is a recombinant tissue-nonspecific alkaline phosphatase fusion protein used as enzyme replacement therapy for patients with perinatal/infantile- and juvenile-onset hypophosphatasia. It targets deficient alkaline phosphatase activity involved in bone mineralization. Treatment is disease-specific and requires specialist monitoring for skeletal response, injection-site reactions, hypersensitivity, ectopic calcification, and laboratory assay interference.
DOSAGE -
Dosage should be determined only by a qualified healthcare professional. Labeled regimens are weight-based subcutaneous injections; common schedules include 6 mg/kg per week given as 2 mg/kg three times weekly or 1 mg/kg six times weekly, with escalation in selected perinatal/infantile-onset patients if clinically required. Injection volume, vial strength, and dose adjustment must follow the current label.
SIDE EFFECTS–
- Possible or reported side effects may include:
- Injection-site reactions such as erythema, pain, pruritus, swelling, or lipodystrophy
- Hypersensitivity including anaphylaxis
- Ectopic calcifications such as ophthalmic or renal calcification
- Fever or irritability
- Headache
- Vomiting or gastrointestinal symptoms
- Laboratory assay interference involving alkaline phosphatase-linked tests
- Antibody development or reduced clinical response
ALLERGIC REACTIONS -
Serious hypersensitivity, including anaphylaxis, can occur.
Watch for hives, swelling, dyspnea, wheeze, hypotension, severe dizziness, or widespread rash.
DRUG INTERACTIONS-
Use caution with:
- Other medicines affecting calcium, phosphate, vitamin D, or bone metabolism
- Concomitant biologics with hypersensitivity risk
- Laboratory assay interference may occur with alkaline phosphatase testing
- Patients should inform healthcare providers about all prescription medicines, over-the-counter medicines, herbal products, and supplements they are using.
PREAUTIONS–
- Confirm diagnosis and onset category.
- Monitor weight, growth, skeletal symptoms, kidney ultrasound/eye exams when indicated, and injection sites.
- Educate caregivers on injection technique and storage.
WARNINGS –
- Serious hypersensitivity including anaphylaxis can occur and requires immediate discontinuation and treatment.
- Ectopic calcification monitoring, including ophthalmology and renal imaging when indicated, may be required.
- Use only under clinicians experienced in hypophosphatasia and metabolic bone disease.
- Keep out of reach of children.
- In case of overdose, severe reaction, or clinical deterioration, seek immediate medical attention.
