ARLOCABTAGENE AUTOLEUCEL
Generic Name: ARLOCABTAGENE AUTOLEUCEL
ABSTRACT -
Arlocabtagene autoleucel is an investigational autologous chimeric antigen receptor T-cell therapy directed against GPRC5D, a target expressed on malignant plasma cells. It is being studied for relapsed or refractory multiple myeloma. The product is manufactured from the patient’s own T lymphocytes after leukapheresis, engineered ex vivo, expanded, and infused after lymphodepleting chemotherapy.
DOSAGE -
- No approved universal dosage regimen is established.
- Clinical trial protocols define leukapheresis, bridging therapy, lymphodepletion, target CAR-positive cell dose, inpatient/outpatient monitoring, and follow-up.
- Doses in CAR T trials are expressed as a specified number of CAR-positive viable T cells and should never be converted to conventional mg dosing.
- Premedication, infection prophylaxis, and rescue therapy must follow the protocol.
SIDE EFFECTS–
- Cytokine release syndrome with fever, hypotension, hypoxia, or organ dysfunction
- Immune effector cell-associated neurotoxicity syndrome, confusion, tremor, seizure, or encephalopathy
- Cytopenias, infections, hypogammaglobulinemia, bleeding, or fatigue
- Nausea, diarrhea, electrolyte abnormalities, or liver enzyme elevations
- Macrophage activation syndrome/hemophagocytic lymphohistiocytosis and prolonged cytopenias are serious risks
ALLERGIC REACTIONS -
- Infusion reactions and hypersensitivity to cellular product components can occur.
- Symptoms include fever, chills, rash, dyspnea, hypotension, facial swelling, or anaphylaxis.
- Emergency treatment must be available at infusion.
DRUG INTERACTIONS–
- Corticosteroids, immunosuppressants, chemotherapy, anti-cytokine medicines, and live vaccines require protocol-directed management.
- Other myeloma therapies may overlap in cytopenia, infection, neuropathy, or organ toxicity.
- Serologic testing and vaccine responses may be altered after B/plasma-cell directed therapy.
- Avoid live vaccines during immune suppression unless cleared by a specialist.
PREAUTIONS–
- Confirm eligibility, organ function, infection status, disease burden, prior therapies, and caregiver/logistics before infusion.
- Monitor for CRS/ICANS at trained centers with tocilizumab and emergency care available.
- Long-term follow-up is required for delayed cytopenias, secondary malignancies, infections, and persistence of engineered cells.
- Pregnancy and breastfeeding safety are not established.
WARNINGS –
- Potentially life-threatening CRS, neurotoxicity, infections, and cytopenias require urgent management.
- Use only in authorized clinical-trial or specialist cellular-therapy centers.
- Patients should carry treatment identification and seek urgent care for fever or neurologic symptoms.
- Keep all biologic handling under controlled medical conditions.
