CARUMONAM
Generic Name: CARUMONAM
ABSTRACT -
Carumonam is a synthetic monobactam antibiotic with potent activity against aerobic Gram-negative bacteria, including Pseudomonas aeruginosa and Enterobacteriaceae. It inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), particularly PBP-3, disrupting peptidoglycan cross-linking. Developed as an alternative to aztreonam, it demonstrated clinical efficacy in urinary tract, respiratory, and abdominal infections in Phase II/III trials.
DOSAGE -
1–2 g intravenously or intramuscularly every 8 hours for moderate to severe Gram-negative infections; dose adjusted in renal impairment.
SIDE EFFECTS -
- Diarrhoea
- Nausea
- Injection site reactions
- Elevated liver enzymes (ALT/AST)
- Skin rash
ALLERGIC REACTIONS -
These symptoms may indicate an allergic reaction and require immediate medical attention.
- Cross-reactivity with other beta-lactams possible
- Urticaria
- Anaphylaxis
- Angioedema
DRUG INTERACTIONS-
- Probenecid reduces renal tubular secretion — increases carumonam levels
- No significant pharmacokinetic interaction with aminoglycosides
- Monitor renal function with concurrent nephrotoxic agents
PREAUTONS–
- Dose reduction required in renal impairment (CrCl <30 mL/min)
- History of beta-lactam allergy — use with caution
- Superinfection with resistant organisms or Clostridium difficile may occur
- Monitor hepatic enzymes with prolonged use
WARNINGS –
- Clostridium difficile-associated diarrhoea (CDAD) reported — can be severe
- Cross-allergenicity with other beta-lactams not fully excluded
- Not active against Gram-positive organisms or anaerobes — confirm culture sensitivity
