DABIGATRAN
Generic Name: DABIGATRAN
ABSTRACT -
Dabigatran is the active direct thrombin inhibitor formed from the oral prodrug dabigatran etexilate. It inhibits thrombin-mediated clot formation. Dabigatran itself is not usually administered as the oral prodrug is used clinically.
DOSAGE -
- Clinical dosing is based on dabigatran etexilate product labeling, not free dabigatran API alone.
- Dose depends on indication, renal function, age, bleeding risk, and interacting medicines.
- Do not start or stop anticoagulation without medical supervision.
SIDE EEFECTS -
- Bleeding, bruising, epistaxis, gastrointestinal bleeding, dyspepsia, abdominal pain, or nausea may occur.
- Anemia, hematuria, or wound bleeding may occur.
- Hypersensitivity reactions are uncommon but possible.
ALLERGIC REACTIONS -
- Immediate rash, hives, itching, flushing, or generalized skin reaction
- Facial, lip, tongue, or throat swelling
- Wheezing, chest tightness, shortness of breath, or severe dizziness
- Severe hypotension, collapse, or anaphylaxis requires emergency care
- Any suspected allergy should be documented and reported to future healthcare providers
DRUG INTERACTIONS-
- P-gp inhibitors or inducers can alter dabigatran exposure.
- Antiplatelets, NSAIDs, thrombolytics, SSRIs/SNRIs, and other anticoagulants increase bleeding risk.
- Renal impairment substantially affects exposure.
PREAUTONS–
- Assess renal function, bleeding history, age, weight, procedures, and pregnancy status.
- Plan perioperative interruption and restart carefully.
- Store and handle according to product labeling to preserve capsule integrity when applicable.
WARNINGS –
- Major bleeding, spinal/epidural hematoma risk, and thrombotic risk after premature discontinuation are serious concerns.
- Urgent care is needed for severe bleeding, black stools, neurologic deficits, or head injury.
- Keep out of reach of children.
